The Information that is required is standard and can be customised slightly. If, however you want to have a bespoke card then please contact us with what you wish to have on the reverse. We will examine your requirements and reply by email with a price.

Medical Condition   You do not need to input details here, if you have no medical conditions. After all this is an Emergency contact number card. If you are putting in medical conditions please list in severity order. All the conditions may not fit on the card.

Allergy/Other     If you are taking prescribed medication think about placing it here along with the dosage. You may also wish to put any requests, e.g. That you are a donor etc.

*1^st Contact Name and Phone number, Their relationship to you and also their mobile number.

2^nd Contact name, and number, their relationship to you and their mobile number.

You will also be asked for the icon of your choice , These can be , No icon, Red Cross or Rod of Asclepius, ( please see gallery ) or any other emergency icon you think appropriate. However we do advise that you use the default of Rod of Asclepius, The symbol on the top left of this page.

There is a section for notes. This WILL NOT be displayed on the card, just information that you may want to submit to us.

You will be asked to upload a file, this is your picture. We allow the following extensions for the pictures gif,jpg,png,tiff 

You will also be asked at this point for a valid email address and phone number. The reason for this is in case we need to contact you with any queries. This information WILL NOT be displayed on the card.

The payment will be for a PIE Card, this is our abreviation for this product.When you pay you will be asked for your home address and then directed to a upload form.
